Abstract

|
연구목적: 환자 간호필요도에 근거하여 기대 간호시간을 산출하고 이를 실제 간호시간과 비교하였다.
연구방법: 간호·간병통합서비스 중증도·간호필요도 14개 항목을 사용하여 11개 의료기관 20개 병동 재원환자 898명의 간호필요도를 측정하였고, 병동 간호인력 474명이 제공한 간호행위를 24시간동안 10분단위로 관찰하였다. 실제 간호시간은 간호사가 제공한 환자 1인당 직접간호시간으로 측정하였고, 3가지 유형의 간호시간(배치기준, 예정된, 관찰된 간호시간)으로 분석하였다. 기대 간호시간은 의료기관 유형과 간호필요도 항목, 교호작용항을 포함한 선형혼합효과모델을 사용하여 추정하였다.
연구결과: 배치기준 간호시간은 0.92∼2.15시간, 예정된 간호시간은 0.88∼1.95, 관찰된 간호시간은 1.00∼2.40, 기대 간호시간은 0.88∼2.33의 분포를 보였다. 배치기준 간호시간이 기대 간호시간을 충족시키지 못한 기관은 11개 기관 중 8곳이었고, 시간외근무를 포함한 관찰된 간호시간이 기대 간호시간을 충족시키지 못한 기관은 2개 기관으로 감소하였다. 3개 기관은 실제 간호시간이 기대 간호시간을 초과하였다.
결론: 환자의 간호필요도에 근거하여 간호사 배치수준을 결정해야 하며, 간호사의 시간외근무를 최소화하기 위해 배치수준을 높여야 한다.

Purpose: To compare actual versus expected nursing hours based on patients’ nursing care needs.
Methods: The nursing care needs of 898 inpatients in 20 wards at 11 hospitals were measured using the 14 items developed by the National Health Insurance Service (NHIS). Nursing activities from 474 nursing personnel were observed every 10 minutes for 24 hours. Actual hours indicated direct care hours per patient day provided by registered nurses according to 3 types: (1) standard hours based on staffing standards approved by the NHIS, (2) scheduled hours excluding overtime hours, and (3) observed hours including overtime. Expected hours were estimated from the linear mixed effect model including hospital type, nursing care need items and their interaction terms.
Results: Standard hours ranged from 0.92 to 2.15; scheduled hours from 0.88 to 1.95; observed hours from 1.00 to 2.40; expected hours from 0.88 to 2.33. Eight hospitals had standard hours not meeting the expected hours and 2 hospitals did observed hours not meeting the expected hours due to nurses’ overtime. In 3 hospitals, all types of actual hours exceeded the expected hours.
Conclusion: Staffing needs to be determined based on patients’ care needs and to be improved to minimize nurses’ overtime work.
